Reimbursement claim rejected? Why it happens and how to get paid
You paid the hospital yourself, filed the bills, and the insurer said no. Most reimbursement rejections come down to paperwork, timing or a policy clause that can be answered in writing.

Quick answer
First get the rejection or closure letter in writing and note the exact reason. If it is about missing papers or a late intimation, send the missing documents with a short explanation and ask the insurer to reopen the claim. If it is about a policy clause such as a waiting period, pre-existing disease or non-payable items, check the date, the clause and your coverage history, then send a written grievance to the insurer’s Grievance Redressal Officer. If they reject it or do not reply within 30 days, go to Bima Bharosa and the Insurance Ombudsman (free, claims up to ₹50 lakh, within one year).
Why reimbursement claims get rejected or closed
- Documents missing: final bill, itemised bill, discharge summary, prescriptions, investigation reports or payment receipts.
- Claim filed after the time your policy allows (often 15–30 days after discharge for the claim form).
- The insurer was not told about the admission in time.
- A waiting period or pre-existing disease clause was applied.
- The insurer says hospitalisation was not medically necessary.
- A query letter was not answered, so the file was closed.
Closed is not the same as rejected
A claim that is “closed for want of documents” has not been decided on its merits. You can usually send the missing papers and ask the insurer to reopen it. A claim that is “repudiated” has been refused on a stated ground, and that needs a reasoned written grievance.
| What the letter says | What to do |
|---|---|
| Closed / no response to query | Send every requested document in one go, with a covering letter, and ask for the claim to be reopened. |
| Late intimation or late filing | Explain the reason for the delay (emergency, ICU stay, travel) with proof and ask the insurer to condone it. |
| Waiting period / pre-existing disease | Check policy start date, renewals and porting history. See the pre-existing disease guide. |
| Not medically necessary | Get a certificate from the treating doctor explaining why admission was needed, with the clinical notes. |
| Non-payable items deducted | Ask for the line-by-line breakup and compare it with your policy and the standard list of excluded items. |
What to do, step by step
- 1Get the decision in writingAsk for the rejection or closure letter with the reason and the clause relied on. A phone explanation is not enough.
- 2Build the filePolicy schedule, renewal receipts, claim form, all hospital bills with receipts, discharge summary, prescriptions, reports and the insurer’s letters.
- 3Answer the exact reasonReply to the reason the insurer gave, with dates and documents. Do not send a general emotional appeal.
- 4Write to the Grievance Redressal OfficerQuote policy and claim numbers, the amount due and what you want. Keep the acknowledgement.
- 5Escalate after 30 daysIf the insurer rejects the grievance or stays silent, register on Bima Bharosa and file with the Insurance Ombudsman within one year.

How long do I have to file a reimbursement claim?+
Your policy sets the time, often 15–30 days after discharge for the claim form and documents. If you missed it, send the claim anyway with the reason for the delay and proof.
My insurer closed the claim because I did not reply to a query. Can I reopen it?+
Often yes. Send all the requested documents with a covering letter and ask the insurer in writing to reopen the claim. If they refuse, treat it as a grievance.
Can I claim reimbursement after cashless was denied?+
Yes. A cashless denial is not a rejected claim. Pay the hospital, keep every original and file for reimbursement, mentioning the earlier denial.
Where do I complain if the insurer will not reverse the decision?+
First the insurer’s Grievance Redressal Officer, then Bima Bharosa and the Insurance Ombudsman if they reject it or do not reply within 30 days.
