Cashless claim rejected at the hospital? What to do next

A cashless denial at the hospital desk is stressful, but it is usually not the end. Here is what it means, what the insurer must do, and how to still get paid.

Quick answer

A denied cashless (pre-authorisation) request is not the same as a rejected claim. You can pay the hospital, then file a reimbursement claim with all bills within the time your policy allows. Under IRDAI’s 2024 rules, insurers must decide a cashless request within 1 hour and final discharge authorisation within 3 hours. Ask for the denial reason in writing; if the reimbursement is also rejected, send a grievance and escalate to Bima Bharosa and the Insurance Ombudsman.

Why cashless requests get denied

  • Hospital not in the insurer’s network, or not informed in time.
  • Incomplete pre-authorisation form or missing reports.
  • Insurer wants more information on a pre-existing condition.
  • Treatment or room category above policy limits.
  • Policy waiting period still running, or the insurer believes an exclusion applies.

The cashless timelines insurers must follow

RuleWhat it means for you
Cashless decisionThe insurer must decide a cashless request within 1 hour of receiving it.
Discharge authorisationFinal authorisation at discharge must be given within 3 hours. If the delay makes the hospital charge more, that extra is the insurer’s cost, not yours.
Reason in writingYou are entitled to know why cashless was refused. Ask the TPA desk for the denial letter.

What to do, step by step

  1. 1Get the denial in writingAsk the hospital insurance desk for the insurer or TPA denial letter with the reason.
  2. 2Fix what you canIf documents were missing, send them and ask the hospital to resubmit the request.
  3. 3Pay and keep every originalIf you must pay, keep the final bill, itemised bill, discharge summary, prescriptions, reports and payment receipts.
  4. 4File for reimbursementSubmit the reimbursement claim within your policy’s deadline (often 15–30 days after discharge). Mention the earlier cashless denial.
  5. 5Challenge a rejectionIf reimbursement is also rejected, send a written grievance, then go to Bima Bharosa and the Insurance Ombudsman.

Frequently asked questions

Does a cashless denial mean my claim is rejected?+

No. It only means the insurer did not approve direct payment to the hospital at that time. You can still file a reimbursement claim with your bills.

How long can the insurer take to approve cashless?+

Under IRDAI’s 2024 rules, 1 hour for a cashless request and 3 hours for final discharge authorisation.

Can I get cashless at a hospital outside the network?+

Some insurers allow it if you inform them in advance for planned treatment, or soon after an emergency admission. Check your insurer’s current policy; otherwise use reimbursement.

What if I was kept waiting at discharge for hours?+

Note the times. If the 3-hour limit was crossed and the hospital charged you more for the extra stay, include that in your grievance.
