1. Policy & KYC Documents3/3 Done

Patient Photo ID (Aadhaar / Voter ID / Passport)Mandatory

Clear copy verifying patient identity and date of birth.

Proposer PAN Card CopyMandatory

2. Hospital & Medical Records1/4 Done

Original Discharge Summary (Signed & Stamped)Mandatory

Must bear treating doctor’s physical signature, hospital seal, admission date/time, discharge date/time, diagnosis, and treatment given.

💡 RED FLAG: If the summary mentions vague duration like "HTN x 10 years" without proof, TPA will reject for PED.
Indoor Case Papers (ICP) / Nursing & Doctor Daily Notes

Complete day-wise temperature chart, vitals, physician visit logs, and nursing notes.

💡 Crucial for overturning "unnecessary hospitalization" or ICU stay denials.
First Consultation Prescription / OPD SlipMandatory

The doctor’s initial prescription slip stating the first onset date of current symptoms and advising admission.

Operation Theatre (OT) Notes & Surgeon’s Report

Required for any surgical admission: pre-op diagnosis, anesthesia record, surgical procedure, and post-op notes.

3. Bills, Pharmacy & Implants1/4 Done

Must have line-by-line itemization: room rent per day, nursing charges, ICU, OT charges, doctor fees, and investigation costs.

💡 POS charge-slips or UPI transaction screenshots are good supplements.
Pharmacy Cash Memos with Prescriptions & Batch Nos.Mandatory

💡 Every bill must clearly state the batch number and expiry date.
Implant Barcode Stickers & InvoiceMandatory

💡 Without the barcode sticker, TPAs deduct 100% of the implant cost.

4. Diagnostic & Investigation Reports0/3 Done

Diagnostic Reports (X-Ray, CT Scan, MRI, Ultrasound)Mandatory

Signed radiologist reports along with physical films/CDs that justified the hospital admission.

Pathology & Blood Test ReportsMandatory

Complete blood counts, cardiac enzymes, cultures, kidney/liver profiles with numerical laboratory ranges.

5. Bank Details for NEFT Payout0/2 Done

Original Canceled Cheque with Pre-Printed NameMandatory

Cheque from the Policyholder’s account with printed name, account number, and IFSC code for direct claim credit.

Insurer NEFT Mandate Form & Passbook CopyMandatory

Signed NEFT mandate form. If cheque does not have pre-printed name, attach first page of bank passbook / statement attested by bank.

4 Red Flags That Often Lead to TPA Rejections
1. Missing Doctor Signature / Stamp:

Computer-generated discharge summaries without physical ink signatures and hospital seal are rejected immediately as "unverified records".

2. Pharmacy Bills Without Doctor Prescriptions:

Any medicine billed without a corresponding entry in the doctor’s treatment sheet will be disallowable as an unprescribed expense.

3. Missing Implant Barcode Stickers:

If the surgical sticker for intraocular lens (cataract) or heart stent is missing, TPAs reduce the entire implant cost to zero.

4. Casual "Suffering Since" History:

Ensure resident doctors don't casually write "history of hypertension since 5 years" if the patient was only diagnosed 6 months ago. TPA will invoke PED.
